How to Get Started

You must upload the Referral Source information into the Initial intake form, including the information from the court and the referring agency, (Name, phone, email, and address; and if you have the following: submit all notes, completion of DUI screening, and Release of Information).

You must complete the Initial intake Form which is recommended to get the process for DUI Education and DUI Treatment moving quickly. After this, we will contact you to collect payment and complete the DUI Education and DUI Treatment onboarding process.
